Complete Medical Waiver & Consent FormThe safety, wellbeing and development of all goalkeepers is a priority at Matt Gilks Goalkeeping. To ensure training is delivered in a professional, safe and consistent manner, the following policies and requirements apply to all participants.
Please read this information carefully before attending training.
Before attending any training sessions, all goalkeepers must complete the Medical Waiver & Consent Form.
For goalkeepers under 18, this form must be completed by a parent or legal guardian.
The form is completed online and only needs to be submitted once, unless medical details change.
This waiver:
Training cannot take place without a completed waiver on file. Parents or guardians are responsible for ensuring the information provided is accurate and kept up to date.
Your submission is securely stored and helps ensure training is delivered safely, professionally and with appropriate safeguarding in place.
By attending a session, goalkeepers (and parents/guardians) confirm that the participant is physically fit to take part in goalkeeper training, including pitch-based and gym-based activities.
If a goalkeeper is carrying an injury, returning from injury, or has any medical concerns, this must be disclosed in advance. Training may be adapted where appropriate, but the final decision on suitability to train rests with the coach.
Matt Gilks Goalkeeping operates with safeguarding as a priority at all times.
Any safeguarding concerns should be raised immediately.
All goalkeepers are expected to:
Disruptive or unsafe behaviour may result in removal from a session. Repeated issues may lead to suspension of training without refund.
While all reasonable steps are taken to minimise risk, goalkeeper training is a physical activity and carries an inherent risk of injury.
By signing the waiver and attending sessions, participants acknowledge and accept this risk. Matt Gilks Goalkeeping cannot accept responsibility for injuries sustained during training unless caused by proven negligence.
Personal belongings are brought to the facility at the participant’s own risk.
Training blocks are booked in advance.
Specific booking and cancellation terms will be outlined at the point of booking.
From time to time, photographs or short video clips may be taken during training sessions for coaching review or promotional purposes.
Consent for media use is covered within the waiver. If you have any concerns regarding photography or video, please raise this before attending training.
All personal data is handled in line with UK GDPR and data protection legislation.
